Being a PreK Teacher is a unique, challenging, and deeply rewarding role. Required documentation:

1. Bright from the Start Pre-K Registration Form (Click link for a printable copy of the form)

2. Georgia Pre-K Program Roster Information Form (Click link for a printable copy of the form)

3. A “certified” copy (with original signature and seal of the issuing office) of the child’s birth certificate

4. a copy of proof of residency electric or water bill(less than 60 days old with service address), a current automobile tag receipt, or voter registration card. 

5. GA Immunization Form 3231

6. GA Dental/Hearing/Vision/Screening Form 3300

7. a copy of the child's Social Security card 

8. If applicable, verification of:

   Medicaid, PeachCare, SSI, Food Stamps, TANF, and/or CAPS program
